The mind and body are intricately connected. I have experienced this in simple ways. For example, I know that when I go for a jog, my mood is lightened and I find it easier to concentrate. Running increases serotonin and dopamine levels, but also releases endorphins, hence the expression, ‘a runner’s high.’
This is a good example of how the physical self is directly linked to mood and attitude.
Now, there is an increasing body of research suggesting that certain dispositions and behavioural tendencies are linked to physical conditions.
The National Library Of Medicine For The National Centre Of Biotechnology Information gives an example, saying that high neuroticism and low conscientiousness are independently associated with increased risk of cognitive impairment and dementia. “It is interesting to note that relative to the other traits in the Big Five model of personality (i.e., extraversion, openness, agreeableness), neuroticism and conscientiousness are the two traits that consistently have been related to Alzheimer’s Disease risk.”
As medicine moves towards increasingly personalised care, an intriguing question is emerging. Could aspects of our personality one day become recognised diagnostic biomarkers?
Personality has traditionally been the domain of psychologists. Physicians have concentrated on measurable biological indicators, such as blood glucose, blood pressure, inflammatory markers and imaging findings.
However, the expanding body of research, such as that indicated above, reveals that personality traits may influence far more than behaviour alone. They appear to affect disease risk, treatment adherence, recovery, healthcare utilisation and even longevity.
While hospitals are unlikely to start screening patients for conscientiousness or neuroticism alongside cholesterol anytime soon, researchers are now exploring whether psychological profiles could eventually become another layer in precision medicine.
The Mind and Body Are More Connected Than Once Believed
The relationship between mental health and physical disease has long been recognised. Depression increases the risk of cardiovascular disease, chronic illness can affect emotional wellbeing, and stress has measurable effects on immune function.
What is changing is researchers’ understanding that stable personality traits themselves may influence long-term health outcomes.
A major 2026 study published in the Journal of Psychosomatic Research examined more than 4 700 adults and found associations between maladaptive personality traits and numerous chronic conditions, including cardiovascular, neurological and rheumatic diseases.
The researchers also found links with higher body mass index, smoking, increased healthcare use and greater overall disease burden. They concluded that the findings ‘support considering personality assessment as part of comprehensive somatic healthcare.’
Rather than suggesting personality causes disease directly, scientists believe these traits may influence the biological pathways and health behaviours which shape disease over many years.
I personally know people who are highly conscientious and militant in sticking to their fitness and healthcare routines. Others are more easy-going and scatterbrained in their approach to healthcare. I know people who are highly anxious and who live with high levels of stress, even when their lives are going well. Others take things as they come. Some people avoid going to the doctor and facing symptoms. Some people open up easily when they are going through challenges, and so their stress burden is less. They’re the ones to seek a doctor’s opinion when they feel something is off with their health.
These behaviours are all functions of personality.
Someone with high conscientiousness may be more likely to attend screening appointments, follow medical advice, exercise regularly and manage chronic illness effectively.
Conversely, individuals with higher levels of impulsivity or chronic anxiety may delay seeking medical attention, struggle with medication adherence or experience prolonged physiological stress which contributes to inflammation and cardiovascular risk.
These behavioural differences ultimately influence the clinical picture appearing on diagnostic images.
For this reason, future diagnostic systems may treat personality as another measurable health variable alongside blood biomarkers and imaging findings.
Artificial Intelligence Could Connect The Dots
One of the greatest challenges is the sheer complexity of the data involved. Genetic profiles, imaging studies, laboratory values, wearable sensor data and behavioural information each generate enormous datasets.
Artificial intelligence is uniquely suited to identify relationships which human clinicians might never recognise.
Rather than examining one risk factor at a time, machine learning systems may eventually discover combinations of biological and behavioural characteristics which predict future illness far more accurately than existing models.
The implications extend beyond diagnosis. Two patients with identical imaging findings may require very different treatment strategies depending on how they engage with healthcare.
Someone who is highly organised and motivated may succeed with home monitoring and lifestyle interventions. Another patient may benefit from additional clinical support, behavioural coaching or more frequent follow-up appointments.
Researchers are increasingly recognising that successful healthcare depends not only on biological precision but also on behavioural precision.
The Future Of Precision Diagnostics
More than likely, the future of diagnostics and treatment will involve a consideration of biological, behavioural and environmental factors.
Personality could very well become another piece of the clinical puzzle, helping physicians understand why diseases develop differently, why treatments succeed for some patients and fail for others, and how healthcare can be tailored more precisely to each individual.
